Information for Referring Veterinary Surgeons

Legally Physiotherapists cannot assess or treat an animal unless we have permission from the animals registered Veterinary Surgeon (Veterinary Surgeon's Act, 1966).

For my records, I ask for written consent from the treating vet to ensure I have an accurate history/ diagnosis, and for legality. The form can be found below, if you wish to refer your client to me directly, please fill out the form and email it to me at everlastingphysiotherapy@gmail.com 

Alternatively, you can ask your clients to contact me, and I can send you a copy of the form to fill out. I do appreciate you are very busy, but I hope by working together we get the best outcomes for our clients.

I aim to send regular feedback (if requested) so we can work as a multi- disciplinary team. I am always at the end of the phone if you wish to have a professional discussion about our mutual clients. 

You can email me to request some leaflets if you would like some leaflets for your reception desk.
