Information for Referring Veterinary Surgeons

Legally Physiotherapists cannot assess or treat an animal unless we have permission from the animals registered Veterinary Surgeon (Veterinary Surgeon's Act, 1966).

For our records, we ask for written consent from the treating vet to ensure we have an accurate history/ diagnosis, and for legality. The form can be found below, if you wish to refer your client to us directly, please fill out the form and email it to us at everlastingphysiotherapy@gmail.com

Alternatively, you can ask your clients to contact us, and we can send you a copy of the form to fill out. We do appreciate you are very busy, but hope by working together we get the best outcomes for our clients.

We aim to send regular feedback (if requested) so we can work as a multi- disciplinary team. We always at the end of the phone if you wish to have a professional discussion about our mutual clients.

You can email us to request some leaflets if you would like some leaflets for your reception desk.
